    REQUIRED DOCUMENTS FOR PPM/PCS REIMBURSEMENT

    To receive reimbursement for a Personally Procured Move (PPM), be sure to gather and submit the following documents to your local Transportation Office or through the Defense Finance and Accounting Service (DFAS):

     

    Must-Have Documents:

  • DD Form 2278: Application for Personally Procured Move
    (Completed and signed by you and your counselor before your move starts) Download DD 2278
  • DD Form 1351-2: Travel Voucher/Subvoucher (Used to request reimbursement after your move) Download DD 1351-2
  • Copy of PCS Orders: Must include all pages.
  • Certified Weight Tickets: One full (gross) weight and one empty (tare) weight ticket, both certified and legible. Tape each to a full-size piece of paper with your name and shipment ID.

    Additional Supporting Documents (if applicable):

  • Paid Rental or Contract Agreements: For moving trucks, trailers, or shipping services like uShip.
