However, delivery of the payment was delayed. The mover has made every attempt to reschedule; however, we still do not have a definite on scheduling this move.

Carolina to Hpuston, Tx. Response from astro64: He is right. I filled it out from S. Carolina to Houston, Texas but somewhere I must of made a mistake that made the shipping appear to need to be shipped from my address to my address. I'm not sure what happened.
